Burnaby FC Evaluation Request for U11-U18 players

Burnaby FC Website is under construction, in the meantime please use this BFC EVALUATION REQUEST to request an Evaluation for one of our Programs. Please fill in all fields so that we know which Level you are currently playing as well as the Level you want to be evaluated for.

Once you have completed and submitted a BFC EVALUATION REQUEST Form (below), we will place you on our Notification list so that we can inform you of any Evaluation dates and or notify you of a day and time to attend a session(s) for assessment.
